NYS Breastfeeding Friendly

Child Care / Day Care Homes

**Partners are not expected to begin this process without assistance and resources from the BFREE Team. The BFREE Team will assist all childcare providers in completing a required Creating Breastfeeding Friendly Communities (CBFC) assessment.**

CHILD CARE CENTER: Complete this self-assessment form.

 

The self-assessment form lists six standards with best practices. The best practices are provided to help your center meet the standards. When you can answer "Yes" to all six standards and include a copy of the center’s breastfeeding policy, your center will be awarded a certificate that designates them as Breastfeeding Friendly on the CACFP website. This certificate expires three years from the issue date and you will need to complete a renewal form at that time.

DAY CARE HOMES: Complete this self-assessment form.

​

The self-assessment form lists six standards with best practices. The best practices are provided to help you meet the standards. Answer Yes to item #7 to have your day care home advertised as Breastfeeding Friendly on the CACFP website. This certificate does not expire.
